Documents required

Listed before the fee, so you can confirm that the required evidence is available before paying.

What is required

Only 2 documents. Everything below is optional, and a submission is complete without it.

  1. Legal registration evidence Upload the current official registration document. Files are held as private portfolio evidence, not published. PDF or image · up to 12.0 MB
  2. Diplomas and qualification certificates of the clinical team At least the clinical lead. Attach as many as you wish; a scan or a photograph is enough. PDF, image, Word or ZIP · up to 12.0 MB
Helpful, but not required 30 documents

A submission is complete without these. Each one gives the reviewer something to credit rather than something to assume.
