What is required

Only 3 documents. Everything below is optional, and a submission is complete without it.

  1. Government-issued identity document Passport or national identity card. You may redact the document number, but the name, photograph and date of birth must remain visible. PDF or image · up to 12.0 MB
  2. Diplomas, qualification certificates and transcripts Every award relevant to your practice, including the initial degree and any further training. Attach as many as you wish. A translation is only needed where the original is in a script the reviewer cannot read. PDF, image, Word or ZIP · up to 12.0 MB
  3. Verified log or institutional statement of supervised training practice It must identify the training provider, period, client-contact hours, supervision hours and signatory. Remove all client identifiers. PDF, Word or Excel · up to 12.0 MB

  • Current registration or licence evidence
  • Recent certificate or letter of good standing
  • Official decision or correspondence about conditions or proceedings
  • Current professional profile, scope statement or service description
  • Current supervision agreement or supervisor confirmation
  • CPD log and completion evidence for the last three years
  • Ethics and professional conduct policy
  • Blank client agreement and consent materials
  • Safeguarding policy and latest training evidence
  • Current professional indemnity certificate and schedule
  • Privacy notice and data-protection procedure
  • Records management, retention and breach procedure
  • Referral, risk assessment and crisis escalation procedure
  • Complaints and appeals procedure
  • Decisions, closure letters or other official supporting evidence
  • Blank outcome tools and anonymised aggregate review
  • Anonymised reflective practice log from the last 12 months

Where a public record exists, the submission asks for its link instead of a copy. Sections can be saved before they are complete.
